Follow these simple steps to print the

wait list agreement:

1. Click on the form to the right.

2. Your computer will automatically open/download the document.

3. The form can be filled in from your computer or you can print the form and fill out by hand.

4. After filling out the form, mail it along with a check or money order for $50 (made payable to Yellow Retirement), to: Yellow Retirement Communities
   P.O. Box 11311 
   Fort Wayne, IN 46857. 

5. We will call you to confirm we have received your agreement and understand your specific needs.
